Position Overview
As a Community Worker, you will play a crucial role in providing essential support services to clients, improving their quality of life, and promoting inclusion within their communities. Your responsibilities will include:
- Developing, planning, and implementing support and welfare programs, particularly for individuals with disabilities.
- Conducting needs assessments to determine the most appropriate service delivery for each client.
- Liaising with families, government agencies, and service providers to ensure coordinated and comprehensive care.
- Monitoring client outcomes and revising support strategies to improve quality of care.
- Advocating for client access to social, healthcare, housing, education, and employment services.
- Assisting clients through transitions and supporting integration into inclusive community environments.
- Maintaining accurate and up-to-date case records and preparing regular progress reports.
- Organizing community workshops and engagement initiatives to build capacity and foster inclusion.
- Referring clients to specialist services where necessary, including housing, mental health, and employment services.
- Promoting awareness of available services and strengthening community networks.
Qualifications and Experience
To be considered for this position, you should possess:
- A relevant qualification such as a Diploma in Community Services or equivalent under the Australian Qualifications Framework.
- A minimum of one year of full-time relevant experience in community work, preferably in the healthcare, disability, or social services sectors.
- Proven ability to assess individual needs and implement personalized support programs.
- Strong communication skills to effectively liaise with clients, families, and service providers.
- Sensitivity and understanding of individuals with diverse and complex needs.
- Ability to work independently and within a multidisciplinary team environment.
- Knowledge of community networks, local services, and NDIS frameworks.
- Cultural competency and respect for diversity within the community.
